The Challenge
A model mechanism for home support and monitoring for citizens with chronic diseases that require systematic monitoring, utilizing modern online services. Each eligible citizen receives remote digital services for systematic monitoring of biomedical parameters and adherence to medication via a smart device.
The application aims to provide immediate, easy and continuous monitoring of patients by obtaining biomedical parameters from measurements that will be performed automatically. These measurements will be taken through a “smart device”, which incorporates a set of special sensors (oximeter, blood pressure monitor, pulse meter, cardiograph, glucose meter, etc.) and will be sent to the central system for monitoring by their doctor. The patients’ doctors will have access to the services, who in turn will be able to use these services to monitor their patients.
